A Moderation ban is any ban authorised by Moderation to perform. These are the majority of bans. Griefing, Pugsy, Raid Rule Breaks, Toxicity, etc. They're the bans you would expect the average player to get if they were to get infractions. TOS bans are a type of Admin ban (any ban severe enough it has to be handled by admins). These are, as it sounds, given out if a player breaches the server's Terms of Service. I believed Terms of Service bans are split into two categories: Player Conduct Violations and Safety Violations.

But the things listed as causes for moderation bans, players can appeal for on the forums, not forced to do so through lotc email like TOS bans are.
