He probably got angry right off the bat for your bad formatting and the fact that you didn't include the questions when you answered them.

Also, you really didn't expand or elaborate on any of the questions. The GM's want to see what kind of person you are and will be. They want to see hat you will add something to the server.

Now, the questions "Biography", "Ambitions", and "character description/personality" are the most important questions. You really need to flesh out your character and make it seem interesting. There are several application guides and example applications on this forum.
