Removed From Whitelist?

While trying to log onto the Lord of the Craft, it said I was not whitelisted.  My whitelist application was accepted back in September of 2014, and I have played on the server a lot.  Here is my application.  Please review the whitelist and add me back on if I am not already.  My IGN is HammerHoops21.
