I find it hard to believe that a plaque can cause sickness... a plaque is a "flat ornamental plate or tablet fixed to a wall, used to mark a significant event, person, etc." The word you were looking for was "plague", haha. I guess it adds realism, printing malfunction, etc. Good job!

I used it in English too, like three years ago! That's where I found it, and I just remembered it. But I think it works really well, and comes out looking nice. As for the font, I tried to make it look incredibly formal, but you are write, it might be a little to print-like.
