Trust me, things have a tendency to look like a hack when it is the opposite.

 

Hacks have a tendency to cause glitches in graphics but a glitch does too, this is merely a death glitch where he dies but somehow the death menu and death effects do not pop up, this is true undeath in the script sense.

 

If he is still like this have a GM reset his player.dat and fix the glitch.

Edit: Ah i did not see those white lines, that looks like a hack Team AVO used, if i were a GM i would ban those guilty of using it.
