I'd say that nothing really drives innovation here. We have monks to rise the dead and clerics to magically regenerate maiming and mutilation. Magic would be more efficient than early musketry if it were developed, and so on.

I'd say LoTC is in an alternate world where there was no purpose in advancing combat or medical technology, so other things were developed in their stead.
